CLEVELAND—An attorney representing the holder of the winning $52 million Mega Millions ticket from the August 20, 2004 drawing came forward to claim the prize in trust, said Ohio Lottery Commission Deputy Director of Communications Mardele Cohen.
Dennis J. Siciliano is the trustee of The ABC Blind Trust, which claimed the $52 million prize. The trust will receive $30,546,540 before mandatory federal and state taxes totaling 28.5% are withheld. After taxes, the trust will receive a check for $21,840,776 since the discounted lump sum payment option was selected as the form of payment.
The holder of the winning ticket personally selected the numbers. The numbers were 12, 17, 34, 37, 39, and the Mega Ball was 34. The winning ticket was sold by Shaker Quality Foods in Shaker Heights, Ohio. Shaker Quality Foods earned a $52,000 bonus for selling the winning ticket.
This is Ohio’s third Mega Millions jackpot winner since December 2003. Mega Millions, a lotto-style game, is drawn twice weekly on Tuesday and Friday evenings at 11 p.m. Odds of hitting the jackpot in Mega Millions are 1 in 135,145,920.
